You're the cover officer that just arrived and the female officer is covering the door so you can move and take up a position of advantage. You take a look around and start to move. Just as you leave cover and expose yourself trusting that she will lay down a hailstorm of suppressive fire if needed, your eye catches something. As you advance into the open your eyes focus on the magazine in her AR-15. An AR-15 magazine doesn't curve back toward the officer does it?? Oh Shit!!!!

Conclusion.
This tactical error could have very well gotten the other police officers killed, and/or  bystanders. Thankfully no shooting occurred, as the lives of others depended on this officer.
MY only problem is with the Conclusion, unless having your head up your ass can be considered a "Tactical Error"

Not having inserted a magazine backwards into my rifle before, I wonder how it managed to stay in there...
There must be enough force on the magazine catch rubbing the side of the magazine that a (hopefully) fully-loaded 30 round magazine would stay in place even when inserted backwards - even though it would be useless.
Eugene Stoner may be spinning in his grave right now, saying to himself, "I never would have thought that a magazine would be inserted backwards like that, so why would I have designed to prevent it?"
